If you have experienced an issue with one of our entertainment kiosks and want to request a service call or a refund, please let us know by filling out the form below. We strive to answer all inquiries within 2 business days. If you need a more immediate response, please call our customer service line at 866-902-0595 or use the live chat function during normal business hours.

Be sure to include your full contact info (name, mailing address, contact phone #), the business name and address where the incident occurred, and the machine type that malfunctioned. Refunds will not be processed unless all information requested is provided.

Refund amounts will not exceed the cost of one play.

If you prefer, written requests for refunds can also be sent directly to the National Entertainment Network corporate offices located 246 S. Taylor Ave, Suite 200, Louisville, CO  80027, Attention: Customer Refund Requests.

All written requests for refunds must include all of the following information in order to be processed:

  • Requestor’s full contact info (name, mailing address, phone)
  • Incident information, Business name, Business address,  and machine type that malfunctioned.
